• The baby's face should fill 2/3 of the photo
• If you have a high quality DIGITAL photo, please upload it to the form below.
• If you have a PRINTED photo:
  • On the back of the photo: Last Name, First Name written on the back in pencil.
  • In an envelope with your FIRST and LAST NAME written on it.
  • Drop off at the office.

2) Grad Write-up

• Maximum 300 characters & spaces combined.
• Please ensure your write-up is school appropriate. Write-ups may be edited if needed.
