Thank you for helping our students attend field trips and/or extra-curricular events by being a volunteer driver.  The form simply provides you with extra insurance coverage by our district and is good until the end of June.

Please download, print, and complete.  Once completed, please bring to the office for an administrator to sign-off prior to your first time driving our students.
